| The Patient form is the central place to control all
information related to the patient. The patient's name is typed in, then by clicking the
patient information button, you can enter additional information. The Insurance button
will open a form to allow for input of the patient's insurance data. The middle of the form includes an area to record patient visit information. You can enter as many visits per patient as needed. General visit information is typed on this form; including date, time, chart ordered, and additional comments.
